support community connection activities for members.
Key Responsibilities
Connect members to community resources to support their care
management journey
Support the care team in identifying member community support and
providing health education
Ensure the provision of necessary care resources in a
cost-effective manner
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
Must reside in the state of Florida
