Job Description:

We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team in Forty Fort, PA. As a Home Health RN, you will provide high-quality patient care and develop meaningful relationships with our patients and their families.Key Responsibilities:

• Provide comprehensive nursing care to patients in their homes, as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Coordinate total plan of care with other healthcare professionals involved in care and help achieve and maintain continuity of patient care.
• Make initial and/or comprehensive nursing evaluation visits, determine primary focus of care, and develop the plan of care within state-specific guidelines.Requirements:
  • Current RN licensure in the state of practice
  • Current CPR certification
